Eligibility 

You are eligible for Nurse Aide Competency Evaluation testing if you are a: 

  • Person who has completed an Indiana State Department of Health approved 105 hour nurse aide training course (more)
  • Nurse Aide Certified in another state (more)
  • Nursing Student (more)
  • Graduate Nurse who has not yet taken State Board Examination (more)
  • Graduate Nurse who has taken but did not successfully pass State Board Examination (more)
  • Foreign Nurse (more)
  • Psychiatric Attendant or Developmental Skills Technician (more)

Nurse Aide Certified in Another State:  

Qualifications:
Must be currently on the nurse aide registry in another state with no verified complaints and have worked at least one 8 hour shift within the past 24 months in that state 

Required Documentation: 

  1. Application: Original Indiana State Department of Health Nurse Aide Competency Evaluation application (complete sections I and IV and sign and date the bottom of side one)

  2. Proof of Certification: Copy of current out-of-state certification which shows the registry number and expiration date
    • CNAsfrom Illinois or Alabama are also required to submit a letter from their employer in that state that indicates they have worked at least one eight hour shift in the past 24 months. The letter must be on letterhead and include the candidate's name, work title, and dates of employment
    • Employers who apply for out-of-state candidates must include a completed Out-of-State Validation Statement on facility letterhead, signed by the facility representative who contacted the appropriate out-of-state registry

Fees

All fees must be paid at the time of registration by money order, facility check, Visa, or MasterCard. Personal checks are not accepted.

Special Needs

Professional Resources will accommodate any qualified candidates with a diagnosed special need.

When applying to test, the teaching entity, candidate, or employer must notify Professional Resources of the candidate's special need. The request to test must include:
  1. Separate letter on letterhead or a note from the candidate that includes the reason for the special testing, and the modified equipment that will be available for use
  2. Copy of documentation from a diagnostician validating the special need

When testing is being scheduled, the teaching entity, the candidate, or the employer should explain to the tester any concerns or considerations pertinent to the testing process.

  • The teaching entity, the candidate, or the employer must provide the necessary modified equipment and the manufacturer's instructions for use
  • Information received by Professional Resources regarding a candidate's special need will be kept confidential

Oral Testing

The teaching entity, candidate, or employer must indicate on the letter of request for testing, the reason for requesting oral testing according to the categories established and approved by the Indiana State Department of Health which include and are limited to:

  • Diagnosed learning disability
  • English as a second language with primary language specified
  • Visual impairment
  • Difficulty with literacy

Cancellation of a Scheduled Test

A candidate will be considered cancelled with full credit only when Professional Resources is notified in writing a minimum of two days (48 hours) before the scheduled test date.

For an employer or teaching entity, submit a letter on letterhead signed by an authorized representative:

  • Include candidate's name and scheduled date of testing
  • Request a refund or credit to your account
  • FAX cancellation at least 48 hours prior to scheduled test (FAX number (219) 462-1997)

For an individual candidate, submit a letter that includes your name, address, phone number, and scheduled date of testing:

  • Letters of cancellation must be postmarked at least two days before the scheduled test
  • Fees will be refunded according to Professional Resources policy

If testing is not cancelled at least 48 hours before the scheduled test and the candidate is not present for the scheduled testing, all fees will be forfeited.

On the Day of Testing

Program Directors and designated instructors are strongly encouraged to observe but may not interfere with the testing process.

Candidates must:
  1. be in the test room at least 15 minutes before the scheduled test
  2. be dressed in whatever their employer considers a uniform
  3. bring an official picture ID (e.g. valid driver's license, state ID) which includes the candidate's legal name, photo, and signature
    • the name on the ID must be candidate's legal name, and must match the candidate's name and signature on the application
    • if an official identification is not provided by the candidate, testing will be denied

Once testing begins:

  • candidates who arrive late may not be admitted
  • children and visitors are not permitted in testing areas
  • electronic devices including but not limited to stereos/radios, cell phones, pagers, and PDAs must be turned off
  • books, papers, pamphlets, and training materials may not be used
  • no food or drinks are permitted on or near the table
  • candidates may not chew gum which can be distracting to other candidates

Candidates may be dismissed with full loss of fees if they:

  • create a disturbance
  • use inappropriate language
  • act in a threatening manner
  • give or receive help
  • refuse to perform a task
  • attempt to remove test materials from the test area
  • use notes, books, calculators, or other unapproved materials during testing
  • refuse to turn off and/or remove electronic devices (cell phones, pagers, radios, PDAs, etc.) from the test area

The tester must refuse to test:

  • a person presenting at a scheduled test for whom the tester has no approved application or request to test issued by Professional Resources
  • a candidate presenting at a scheduled test without valid official identification
  • a candidate presenting at a scheduled test with an injury or illness which prohibits her or him from working without restrictions
    • if a candidate is on restricted activity or light duty, that candidate will not be allowed to take the skills portion of the competency evaluation
    • the candidate will be allowed to sit for the written/oral portion of the competency evaluation

Test Scoring

Test questions and skills evaluations are developed directly from the Indiana State Department of Health approved Resident Care Procedures and Course Content Standards.

  • Tests are scored at the testing site
  • Pass/fail results are assigned to both the written/oral and skills evaluation
  • Candidates receive test results at the test site on the day of testing
  • Test results are not revealed by phone
  • If test results are unsatisfactory, documentation provided to the candidate states the test(s) to be retaken and areas of needed study

Hosting a Testing

Teaching sites and facilities may host testing for their candidates and others. The host facility must provide:

  • A well lighted, well ventilated, quiet space for written testing that does not interfere with resident activities
  • A lab set-up with all required equipment and supplies necessary for realistic skills performance

If resident activities are conducted in the testing room, testing must be scheduled at an alternate time to accommodate the residents.

Professional Resources reserves the right to refuse to test at any location which:
  • Is unable to provide space that does not interfere with resident programs
  • Permits interruptions in testing areas that may disrupt testing and breach candidate confidentiality
  • Will not meet policies and/or will not abide by the procedures necessary for testing

 

EQUIPMENT AND SUPPLY LIST FOR NURSE AIDE SKILLS EVALUATION

Signs for doors of testing rooms stating: "Test In Progress" and "Do Not Enter"
 
Volunteer "Resident": Indiana State Department of Health approved program director or instructor who will act as the resident during the testing department. The teaching entity may provide a mannequin if the teaching staff are not available.

Resident Unit: call light or facsimile, privacy curtains, bed with side rails and pillow, overbed table, bedside stand

Infection Control: sink, towels and dispenser, trash can, soap dispenser, gloves, soiled linen hamper, alcohol swabs

Bedmaking, Dressing: two sets of linen including top and bottom sheets, pillow cases, spread, bath blanket, two gowns, slippers, large pants, and button front shirt

Ambulation/Transfer: walker, wheelchair, stationary chair with arms

Bathing/Hygiene: wash basin, soap, towels, washcloths, nail clipper, orange sticks, nail file, tooth brush, tooth paste, mouth swabs, petroleum jelly, lotion, emesis basin, mouthwash, glass, dentures, electric and safety razors, comb, mirror, hair brush

Elimination: bed pan, fracture pan, toilet paper, urinal, urinary drainage bag, paper towels, graduated cylinder, I&O sheet, bedside commode

Vital Signs/Weight: oral thermometer, sphygmomanometer, teaching stethoscope (dual), watch with second hand, access to scale

c. Residents in phase three are of Alzheimer's disease dependent for all ADLs.
d. Residents in phase three are of Alzheimer's disease unable to recognize family, staff, and self.

12. B
Place the chair on the resident's unaffected side (strong side). The unaffected side supports weight.

a. Placing the chair on the resident's affected side (weak side) may put undue stress on that side and result in pain, loss of balance, and possible injury for the resident.
c. The chair should be braced firmly against the side of the bed which helps stabilize the chair and is the shortest distance for the resident to turn.
d. If the resident needs to ambulate (walk), the care plan will reflect that need. The CNA does not determine the resident's need to walk.

13. A
When making an unoccupied open bed, the top linen should be fanfolded to the foot of the bed with the top edge closest to the center of the bed. If the resident gets into the bed, he must be able to reach the edge of the top linen to cover himself.

b. If top linen is not put on the bed, the resident will be unable to cover himself.
c. Fanfolding the top linen to the side of the bed allows the resident to get into the bed from one side only. Fanfolding linens to the bottom of the bed allows the resident to get into the bed from either side.
d. If the top edge of the linen is closest to the foot of the bed, the resident will have difficulty covering himself.

14. C
When walking with a resident, you should stand to the side and slightly behind the resident. This position allows you to clearly see in front of the resident, allows you to guide the resident toward a clear path, and puts you in a position to assist the resident if he begins to fall.

a. Standing in front of the resident limits your ability to clearly see the path and any obstacles that may be present. Holding both of the resident's hands may cause imbalance.
b. Standing behind the resident limits your ability to clearly see the path and any obstacles that may be present.
d. Never attempt to move or stabilize a resident by holding her under the arm. If the resident begins to lose balance or fall, you may cause serious injury to the resident's shoulder and arm.

15. B
Handwashing is the best way to prevent the spread of infection.

a. The CNA must clean up spills immediately to insure resident safety and clean the overbed table after performing procedures but housekeeping is responsible for keeping the resident room clean and sanitary.
c. Residents have basic psychosocial needs including the need for social interaction. Keeping a resident away from others violates the resident's right to have access to visitors.
d. Gloves should be worn whenever there is a possibility of coming into contact with body fluids.

16. D
Remain calm, provide for the resident's safety, and report to the nurse immediately. Difficult behavior may result from too much stimulation, change in routine or environment, physical pain or discomfort, reactions to medications, or fatigue.

a. Refusing to provide care for the resident is neglect.
b. Any action or thing that restricts the resident's movement is considered restraint. It is unsafe for the resident and the nursing assistant to try to physically restrain the resident.
c. CNAs are not allowed to give medication or determine if medication is necessary.

17. A
Reality orientation helps the resident remain aware of who they are, where they are, and a sense of time.

b. Talking about past experiences is reminiscing.
c. Validation therapy helps residents improve dignity and self-worth by having their feelings and memories acknowledged.
d. Ignoring a resident may be considered psychological abuse.

18. C
Offer the resident a choice of two outfits to be certain that the resident is dressed properly. Residents with dementia need assistance when dressing.

a. The resident has a right to choose clothing and the resident's self-esteem is improved when choice is offered.
b. A resident with dementia may be unable to select appropriate clothing or become overwhelmed with many choices. Being certain the resident is dressed appropriately preserves the resident's dignity.
d. The resident's family is not responsible for selecting the resident's clothing on a daily basis.

19. A
A protective device (restraint or safety device) must be applied according to manufacturer's guidelines for safety and legal reasons.

b. Devices with ties should be secured with a quick release knot for safety.
c. The device must be checked for proper fit and comfort by placing the open hand flat between the resident and the restraint.
d. A protective device may be used only with a doctor's order. The CNA should follow the direction of the nurse in charge.

20. B
To communicate with a confused resident, watch the resident's facial expressions and body language for indications of needs, feelings, and moods. Understand that all behavior has meaning.

a. By ignoring the resident's attempt to communicate or by pretending to understand, important information can be lost.
c. The CNA has the obligation to attempt to understand the resident's communications.
d. The resident may become agitated and lose trust and confidence in the CNA if the nursing assistant makes no effort to understand what the resident wants or needs.
